Graft versus host (GVH) is a term used to describe a phenomenon in which a graft (transplanted tissue or organ) attacks the host body, its normal cells and tissue, to which it has been transplanted. It is a form of rejection that occurs when the body's immune system mistakenly identifies donor stem cells as foreign and launches an attack on them.

GVH can occur when the donor tissue or organ is mismatched with the tissue or organ of the recipient, or when the immune system of the host is not able to control the donor's immune cells. Symptoms may include fever, abdominal pain, jaundice, skin rashes, and coma. Treatment may involve immunosuppressive therapy, use of gamma globulin, and other measures to control the symptoms.

GVH can also occur in bone marrow transplants, where donor T cells can cause an immune reaction against the recipient, although this is usually less severe than in allografts.

GVH is a major concern for those receiving transplants, and it is important for recipients to be aware of the risks involved. While treatment is available to minimize the occurrence of GVH and its disruptive effects, prevention is the best way to avoid this potentially deadly complication.
